Mobile Health Team Inc., a staffing agency has an opening for a Hospice RN. We are looking for someone with a passion for people.

Location: Rutland, VT 05701

Assignment Start Date: 11/01/2024

13 weeks | Days | 8-hour shift | 40 hours with 37 hours guaranteed weekly

RESPONSIBILITIES

1. Utilizing the nursing process, carries out a comprehensive assessment of patient/caregiver needs to provide quality care to assigned patients, by State and Federal regulations, Vermont State Board of Nursing, and VNAHSR policies and regulations..

2. Develop and communicate a realistic plan of care for each patient by State and Federal regulations, Vermont State Board of Nursing, and VNAHSR policies and regulations. Ensures that the patient/caregivers are included in the care plan process.

3. Actively participates in IDT meetings to develop an appropriate individualized plan of care within five days of admission, and every fifteen days subsequently, carries out a comprehensive assessment with other members of the IDT of patient needs.

4. Serves as Case Manager for assigned patients and actively manages caseloads and visit times by IDT plan of care.

5. Implements the medical plan of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients by State and Federal regulations, Vermont State Board of Nursing, and VNAHSR policies and regulations.

6. Stay up-to-date on a variety of pain control, symptom management, High-Tech skills; IV use and maintenance, wound VAC, complex wound care, etc. Takes action to ensure that infection control standards are maintained throughout the plan of care.

7. By LPN scope of practice, delegates duties to LPNs to ensure that the plan of care is followed, delegates tasks, and ensures that patient needs are met. Consistently reviews documentation, annotates visit notes, and acts on recommendations by department policies and procedures.

8. Evaluate a variety of aspects of the implemented plan of care including, but not limited to identified problems, care provided, and patient's responses, to meet patient/caregiver needs and ensure overall quality of care delivered, by State and Federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations.

9. Maintains up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team to ensure ongoing and continued quality of care, by evidence-based practice, State and Federal regulations, Vermont State Board of Nursing and VNAHSR policies and regulations.

10. Follows agency protocol with McKesson processes, including but not limited to, locking-down visits, follow-up notes, visit completion, etc.

11. Performs patient and caregiver education to ensure informed patient decisions can be made, including, but not limited to, medication side effects, treatment options, discharge instructions, reportable conditions, ranges of services, etc. by State and Federal regulations, Vermont State Board of Nursing and VNAHSR policies and procedures.

12. Accurately and fill out a variety of Agency, State, and Federal forms, including but not limited to, HHABN, Medicare Notice of Non-Coverage, etc. promptly. Takes action to ensure that forms are readily available during visits to facilitate timely completion.

13. Carries out supervisory visits by State and Federal regulations. Takes action to communicate information to the Scheduler promptly.

14 Assesses patients' needs for HCA/Hospice Aide services, develops and coordinates POC with patient/caregiver, and provides ongoing supervision by State and Federal regulations.

15. As necessary, transmit patient incident reports (infections, injuries, etc.) to QI specialists in a timely manner.

16. Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed by department policies and procedures, and State and Federal regulations.

17. Coordinates a wide variety of patient care with physicians, providers, team members, vendors, etc. in support of the recommended plan of care to ensure safe and effective patient and family care, by evidence-based practice, Vermont State Board of Nursing and VNAHSR policies and regulations.

18. Fulfills agency responsibility to participate in monthly peer review audits. Provides constructive feedback and openly accepts feedback professionally.

19. Before admission, obtain prior authorization for services when needed. Actively engages in ongoing communication with an insurance case manager to provide timely updates on services rendered and/or changes to the plan of care.

20. Develops and carries out educational presentations at various facilities, in-house, and with community and outside agencies.

21. Facilitates transfers of patients to a variety of agencies, facilities, etc. Takes action and completes appropriate documentation to maximize the efficiency of the transfer.

22. Actively participates in bi-weekly team meetings to maximize effective communication with team members. Ensures effective and timely documentation of meetings is maintained.

23. Proactively advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs.

EXPERIENCE/EDUCATION/QUALIFICATION

Experience:

  • A minimum of 2 years of Hospice RN experience preferred.

Licenses, Certifications, and/or Registration:

  • A current active VT State RN license is required

EMR System: McKesson experience preferred

Shift: M-F 8am-4pm. Some weekends and on call are required.
