Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!!   NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). Traveler  must  be willing to work TWO of the following:  
Thanksgiving and weekend after
Christmas Eve/Christmas Day and weekend after
New Year Eve/New Year Day and weekend after

Nonbillable Orientation: 8hrs
Holiday, OT, Call-Back Multipliers: 1.25%
Call: 4.00

3 years current CSFA experience  
NBSTSA REQUIRED (NCCT not accepted)
AT LEAST  TWO  SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW  MUST  BE ADDED TO PROFILE - PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION. "ANYTIME" WILL NOT BE ACCEPTED.  
Traveler must have their own vehicle for this need. This is a firm requirement
Profile Requirements:  
Work History
Skills Checklist (pertinent to position), current to 1 year  
1 Supervisory Signed Reference (from within the past two years of employment)
License/Certifications - Must be uploaded with profile (NBSTSA, BLS - Facility will not pend license/certs)

CERTIFICATION REQUIREMENTS : 
BLS (AHA)
NBSTSA

ADDITIONAL LICENSE REQUIREMENTS : 
Weekend Requirements : Every Other weekend /Must be available for all holidays as/if needed
On Call Requirements : CALL REQUIRED As Needed
