Pride Health is hiring for a Physical Therapist to support our client’s medical facility based in Modesto, CA. This is a 27-week travel contract opportunity and a great way to start working with a top-tier healthcare organization!   Job Details: The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home, working in close coordination with the PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function. The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the rehabilitation department. Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.Supervises the Therapy Assistants, including providing the training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP).Provide training and instructions to staff, participants, and caregivers/families in the use and care of therapeutic equipment.Order, track, and manage maintenance for equipment.In coordination with the Marketing Team, support enrollment of prospective participants into the program.Participate in end-of-life care coordination and support.Manage direct reports, including training, supervising, and mentoring.Apply for specific facility details. Qualifications: Bachelor’s degree in physical therapy (Master’s/ Doctorate Preferred)State CA Physical Therapy License RequiredOutpatient Experience PreferredBLSFive years of progressively responsible PT experience.Minimum of two years’ Physical Therapy experience in a clinical setting with a frail or elderly population.Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.Two years’ supervisory experience with demonstrated ability to mentor and develop team membersAdditional Information: Location: Modesto, CA 95350Job Type: Travel ContractSchedule: 27 week (ASAP)Pay Range/Salary Range: $59/hr - $69/hr  *Offered pay rate will be based on education, experience, and healthcare credentials.
