JOB DESCRIPTION

Position: Certified Surgical First Assist - Operating Room(OR)
 
Start Date: 11/18/2024
 
Duration: 13 Weeks
 
Location: Great Barrington, MA, 01230
 
Shift Type/Shift Times/Hours/Day: Days, 8
 
Shift Notes: 8 Hr Days 6:45a - 3:15p 40hr guarantee
 
Weekend Requirements: Every Other weekend /Must be available for all holidays as/if needed
 
On-Call Requirements: CALL REQUIRED As Needed
 
COVID Requirement: COVID VACCINATION (dose date on or after 10/3/23) NOW REQUIRED FOR THIS HEALTH SYSTEM.  Exemption  forms are available in Downloadable Forms (Both forms REQUIRE Provider/Religious Leader Signatures),  or please ask your delivery team for documents.  IF APPROVED BY THE HEALTH SYSTEM TRAVELER WILL WEAR A MASK AT ALL TIMES WHILE ON SITE.  To avoid delays in processing,  please include the traveler's COVID vaccination card/or intent to fill out the exemption form with profile submission.  
 
Traveler Policy: Candidates' Permanent Address (Facility will not consider local candidates - 50-mile radius restriction)
 
**CORI/SIR SEARCHES ARE REQUIRED FOR THIS POSITION/TO BE STARTED ASAP UPON OFFER ACCEPTANCE/Candidate fills out, Facility conducts  (Candidate cannot start assignment until these searches are resulted).
 
** Requested time off during this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!!   NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). Traveler  must  be willing to work TWO of the following:  
 
** Traveler must be willing to work TWO of the following:  Thanksgiving and the weekend after Christmas Eve/Christmas Day and the weekend after New Year's Eve/New Year's Day and the weekend after
 
  
 Submission Requirements 
 
3 years of current CSFA experience  
 
NBSTSA REQUIRED (NCCT not accepted)
 
Traveler must have their vehicle for this need.  This is a firm requirement
