Job Title:  LVN / LPN - Long-Term Care (LTC)
 Location:  Providence Seward Mountain Haven, Seward, AK
 Start Date:  04/15/2025
 Duration:  13 weeks
 Shifts:  4x12-hour (Days/Nights)
 
 
Job Details:
 Pay Rate:  $50/hr
 Guaranteed Hours:  60 hours per 2 weeks (1 shift cancellation allowed)
 Patient Ratio:  1:20
 Experience Required:  1+ year LTC experience
 Certifications:  BLS, Alaska LVN/LPN License (pending licenses accepted)
 Travel Experience:  Required
 Floating:  Yes, within scope
 Block Schedule:  No
 RTO:  Must be included in submission
 
 Additional Information:
 Housing/Travel:  Facility provides a rental vehicle (restrictions apply)
 Holiday Requirement:  Work 2 out of 3 major holidays (Thanksgiving, Christmas, New Year)
 
 Compliance:
 Two supervisory references required (within the last 3 years)
 Must be 50+ miles from facility (no locals)
 Permanent staff must wait 3 months before returning as travelers
 Maximum 1-year assignment; 6-month break required before returning
 Facility only accepts Hep B vaccines (no titers/declination)
 40 hours (non-billable)
 Modules:  Average 16 hours (non-billable, completed pre-start)
