Summary:  St. Michael Medical Center is Looking for GASTROENTEROLOGY Hospital Coverage to perform procedures, provide inpatient rounding and consultations

Locum will need to get oriented prior to Feb 15 when shift starts.
Bid Due: 11/4

RATES:
Malpractice     Must be included in rates. 
Orientation     half the hourly/callback rate, kicks in after 8 hours.    Per hour of orientation or training beyond 8 hours   
Hourly     xx541 - xx560 -    Per hour, not OT   
Holiday     Must be 1.5x the regular rate.    Per holiday hour, no OT   
Nightly Call     xx586 - xx595 -    Per night call to hold the pager, to include 2.5 hours gratis   
Holiday Nightly Call - xx879 - xx892.50  - "Per night call to hold the pager, to include 2.5 hours gratis   
On Call (Daily)     xx3751 - xx3995 -    Per 24 hours to include 4 hours gratis"
24 Hour Holiday Call Rate -  xx5626.50 - xx5992.50 -    Per 24 hours to include 4 hours gratis"
Call Back     $541 - xx560 -     Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.   
 
Holiday Call Back     xx811.50 - xx840 - "Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.   

Schedule/Availability Requirements:
Shift start and end times? Monday -Friday 8-5pm. Call 1500-0700 weeknights, 24 hr weekends.  
On site - 8-5pm...inpatient rounding, procedures, and on call.  
Off site - after hours...holding the pager and responding to GI urgent needs.

Facility Location: St Michael Medical Center  
Setting: Specify % of procedures vs clinic: NO Clinic  
Duties: 
FTE: full time
Required Procedures: EGD and Colonoscopy  
Average # of procedures/day 6-8
Do you require locums to perform ERCP or EUS? Preferably ERCP
Rounding required? Yes
Patient types: GI Endoscopy patients, adults, inpatients, on call patients
EMR: Epic  
Support Staff: nursing staff
Reason for Coverage: provider retiring
Dealbreakers: No new graduates. Locums who cannot provide 7 consecutive days/mo.  
  
Travel Preferences  (see attachment for specific travel guidelines):
   Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available, hotel/lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market.
