What You'll Do:

As the Implementation Coordinator for the NE TN GROWSS project, your primary responsibility is to serve as the key point of contact for identified practices and support the implementation of tele-behavioral health services. You will collaborate with providers to develop and optimize workflows, support data management, and ensure continuous quality improvement (CQI) processes are met.

Key Responsibilities:
  • Serve as primary point of contact for the Identified Practices.
  • Coordinate with IP points of contact to support approach to integrated tele-behavioral health.
  • Provide support in developing implementation plans, which includes but is not limited to coaching, workflow shifts to increase fidelity, and relationship building with providers.
  • Support data collection, acquisition, and effective extraction.
  • Collaborate to establish quarterly CQI and to support IPs in addressing quarterly KPIs for improvement on CQI metrics.
  • Connect to additional T&TA as needed, such as with ZERO TO THREE and/or HMG!
What We're Looking For:
  • Bachelor's degree in Healthcare Administration, Public Health, or a related discipline (required); Master's preferred.
  • At least 3 years of experience in healthcare settings.
  • Proficiency in Microsoft Office, Google Drive, and familiarity with data evaluation programs.
  • Experience working in a medical or clinical setting is preferred.
Key Skills:
  • Excellent communication skills, both oral and written.
  • Strong organizational skills and the ability to work independently.
  • Experience working in interdisciplinary teams and effectively managing projects.
