Contract Length: 13 weeks-Accepting Local and Travel Candidates
 
Pay Rate: $1400 Weekly 
 
Unit: Home Health
 
Shift: 8 Hours; Days, 8.00 am - 5.00 pm
 
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri
 
Position Requirements:
 
 
LPN experience: 2 years - Required
 
Recent/Current Home Health Nursing experience: 1 year within the last three years - Required
 
OASIS experience - Required
 
Home Care Home Base (HCHB) charting experience - Required
 
Must have valid Driver's License and Personal Auto Insurance
 
Must be Covid Vaccinated or provide medical or religious exemption
 
 
Education, License and Certification Requirements:
 
 
ASN or an equivalent     Required
 
BSN or an equivalent     Highly preferred
 
LPN License for State of Florida or Compact - Required
 
BLS Certification from AHA or American Red Cross - Required
 
 
Job Description:
 
 
Provide and coordinate care under supervision for assigned patients in comfort of their homes
 
Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
 
Empower patients and families to reach healthcare goals by educating them on disease management
 
Ensure patients' status is assessed on an ongoing basis and pertinent information is gathered and documented.
 
Notify physicians of significant changes in status
 
Document and complete charting for all patient visits within 24 hours.
 
Territories to cover: Panama City and surrounding counties
